How to find and edit a hazard

When a hazard has been reported, an email is sent to the assigned person to assess the risk. Once a hazard has been reported it will need to be assessed/reviewed ongoing until it is eliminated, here's how to do that. 

Hazards should be assessed as soon as possible to minimise risk. 

Note: Additional emails or documents no longer need to be filled out, saved or emailed by you or the reporter.  It is now all recorded and saved to a single place that people can access as appropriate. 

Go to the Well eForms Hub, from here, go to the Health and Safety section and click Hazard Risk Assessment

 

To find the hazard to assess, you can use the search bar to search for yourself (This will show you the hazards assigned to you) or you can search for the Hazard ID (found in any hazard system emails you are sent).

If this is an initial assessment of the hazard the status will show as New and the Assess Initial Risk button will show as blue. To assess a risk for the first time, click on Assess Initial Risk

Fill out the details of your assessment, based on the options selected the Initial Risk Rating will be automatically calculated, then click Save
Note: Include any action taken to initially mitigate the risk. 

 

Once a hazard has been initially assessed, you will be able to search for the hazard and Assess Residual Risk as this will now be showing as blue and clickable.

Note: Assign the risk to your team leader, facility lead or admin person to asses the risk. 

Fill out the details along the top with updated information, once you have filled out this information you will be able to change the Hazard Status to reflect the update.

The aim will be to eliminate hazards where possible, or to minimise the hazard by isolating or monitoring. If Isolated or Monitored, you will need to complete the Date of last review. This is to ensure that we are monitoring this effectively.

Access the consequences before the likelihood, consequences is the worst case scenario and likelihood is how often it might happen. 

Residual risk is based on the action plan i.e., once the controls are in place what is the consequence and likelihood.

The review frequency should be based on the risk, if it is a high/critical risk this should be reviewed more frequently. 

 

Hazard status Meaning
New Hazard has not been assessed yet
Assessed Hazard has been assessed - See notes in hazard to see current action plan.
Minimised - Isolated Hazard has been minimised by isolating to reduce the risk - This can be reviewed on a reoccurring basis. 
Minimised - Monitored Hazard has been minimised by monitoring the risk - This can be reviewed on a reoccurring basis.
Eliminated Hazard has been eliminated completely and can be closed off. 
Not a hazard Hazard was assessed and deemed to not be a hazard.


You can see the last reviewed date and set a time frame for the next review of the hazard, this will send you an email to review it based on the timeframe selected.
