Client related incidents such as abuse incidents, medication errors, property damage, theft, privacy breach

Why is this important?
  • Keeps people safe – Shows risks of harm to tangata whai ora (e.g. abuse, medication errors.
  • Meets legal and ethical responsibilities – Ensures the organisation is protecting tangata whai ora rights and meeting standards.
  • Highlights where things might be going wrong – Identifies patterns and system issues.
  • Protects reputation and trust – Serious incidents can damage public trust if not managed well.
  • Supports better decision making – Helps the Board prioritise improvements and resources.
  • Gives the Board confidence that services are safe, ethical, and professionally managed.
