Evidence of NZ Certificate in Health Level 4

Some staff are required to provide evidence of a New Zealand Certificate in Health and Wellbeing (Level 4) 

Acceptable forms of Evidence

• Graduation certificate of Level 4 or equivalent training

• A transcript plus a letter that clearly states the date that they finished

Note: We do not accepts transcripts alone, it does require a letter that clearly states the completed date. 

Qualifications that are in progress can be uploaded once completed. 

Equivalent training

There are training equivalents to the New Zealand Certificate in Health and Wellbeing (Level 4). To see if you have an equivalent training check the Ministry of Health's Level 4: Pay Equity Qualification Equivalencies.

Note: If you have completed your qualification but have not received your graduation certificate, this will be back paid to the graduation date on the certificate once uploaded.
