1. Before you start
You will need your registration number, and evidence of your registration (such as a registration certificate) ready to upload.
This evidence can be a scanned or downloaded file in PDF, Word or image format (png, gif, jpeg).
2. Open the form
Go to Cornerstone Welcome page, and in the My Hub section select Update My Details option.
In the Professional information section choose the Registration and Practicing Certificate option.
3. Complete the form
Enter the details of your registration, and attach the file with evidence (PDF, Word or image file).
4. What next?
First your manager will be sent an email asking them to approve (in Cornerstone).
If this change will result in a change in pay, your manager will be required to complete a variation to employment form.
